Time to hit the Genius Bar at an Apple Store. Your battery may be "consumed" or worn out. Apple can replace the battery with a brand spankin' new one for about $80, even if you don't have AppleCare.

Having a phone that stays on when needed is huge. I apologize for the trouble. We need to figure out if the issue is hardware or software related. If you try settings>general>about> version, what version do you have? How long has it been this way? We will help make things right.BobbyN_VZW

Let's look further into this spjosef. Did anything specific happen right before this began? Have you had a chance to try a different charger/USB adapter? What software version are you running in Settings>General>Software Update? Please share. Thank you.
