where is my Personal Unlocking Key (PUK) located?

i attempted to lock my sim card but i entered my pin wrong more than three times, now i have 10 attempts to enter my puk. does anyone know how to find this?

Hello boyare123! I can help you with the information you need in order to get your iPhone 4S unlocked for use in another country. Are you currently located in the US or are you already out of the country? If out of the country, please call Global Support 800-711-8300 or 908-559-4899. If you are still within the US, here are the requirements to get your SIM card unlocked.

-Must be a VZW customer
-Must be active for at least 60 days
-Must have a current balance
-Must be in good standing for at least the past 60 days (no service interruptions)
