How do I report outstanding customer service?

I was helped a few hours ago by Valerie from Verizon Wireless, specifically for Device Activation on a new device not registered under the current account. Ultimately, I wasn't able to resolve my issue because it was simply a problem with an old AT&T-distributed iPhone 3GS that doesn't properly register under Verizon's device activation. However, Valerie went above and beyond in assisting me and was very patient, too. I only regret that there was no formal way to provide feedback for her help.

Contact one or more of these executives.  Executives will usually make sure the good word reaches the right people and appropriate praise to the employee.
