Missing FedEx Return Label

My iphone 6 (we'll call it phone A) that I purchased in July 2015 developed a hardware issue. I jumped through all the hoops with tech support and Verizon agreed to send me a new phone (phone B) to replace phone A. Phone B arrives, but there's no shipping label in the box to return Phone A. I contact customer support, both over phone (multiple times) and over chat, trying to get a new shipping label sent to me. I finally thought I had gotten somewhere last week, but what I received in the mail was a padded envelope to put the phone in, but NO shipping label.

The only return label I can print from my account is to return phone B, the phone that replaced the defective one. Am I supposed to print that out and send the defective phone in its place?

So, my question is, is there any possible way I can get a new return shipping label for phone A sent to me? Or can I get a shipping address? At this point I am willing to pay FedEx myself.
