Update:

I'm currently on call #6. Total time thus far on this call: 1 hour and 19 minutes.

This latest customer service representative is now claiming that the reimbursement website "has a bug" and that I should go into the nearest Verizon corporate store to complete this reimbursement request submission.

I'm still on hold waiting for a case number and who to contact at this store and who to call if (and this is quite likely) the store claims they have no responsibility for helping me.
