Yes, when you are outside of the US and want to call a US number, you must use the country code. Exceptions would be countries which use area codes similar to US, such as Canada. Then, the 10 digit number should suffice.

If your spouse's number is (123) 456-7890, put the number in your contacts list like this:
+1 123 456-7890
That will work in the US or anywhere you are travelling.
