Second you fill out the information on the phone, submit the documentation, and mail Verizon your old phone. You do not get to keep it. If you don't send back, no gift card. Also the amount is "up to" 650, minus the appraised amount of your phone. If the payment balance in your old phone is 300, and phone is appraised at 100 you only get 200. If Your etf is 250 and appraised amount is 100 then you only get 150. Hope this clears things up.
