Log into your account at verizonwireless.com.  Once logged in, it will show a picture of your phone (the picture may be different if you are not the account owner).  Click on this picture (even if it isn't your phone).  It will take you to anther screen that will display pictures of all the phones on the account.  If you have to, choose the phone that is yours by clicking on the picture. Look below that and a larger picture of the phone will be there.  There will be some text to the right of this picture, look to the end of the text and there should be a link that will show you what you PUK code is.  If it doesn't you may want to call Verizon.
