naansalad: I do meet all terms and conditions. That's not the problem. The problem is Verizon's customer service.

were the numbers ported to Verizon? did the bill you uploaded provide a detailed itemized billing for the device payments with your previous provider?

FFirst of all, let's make something clear. You only get the gift card after 6 months of service. This is on the website. Second you fill out the information on the phone, submit the documentation, and mail Verizon your old phone. You do not get to keep it. If you don't send back, no gift card. Also the amount is "up to" 650, minus the appraised amount of your phone. If the payment balance in your old phone is 300, and phone is appraised at 100 you only get 200. If Your etf is 250 and appraised amount is 100 then you only get 150. Hope this clears things up.

Well...glad to see it is not just me.  I have been having the same issues, submitted it all the way it was suppose to be done.  Got told on 2/8/16 that everything was fixed.  Today got an email stating "Household Max Reached" ?  Okay...now, waiting online for a rep for almost a hour, also on hold with the (844) 408-8470 phone number for 25 mins and finally get rep.  She asks how many phones, nothing about tracking, and puts me on hold?!?  Okay, seriously...Verizon needs to teach the people in the store how to go over terms and limitations, need to do a WHOLE lot better on customer service and their knowledge, as well as the wait times to even get someone to talk to you.

I have the same issue! Submitted my receipt after receiving an e-mail that said I had submitted the incorrect one. I have submitted 3 times myself and two other times by the store manager and an employee there. No luck! Called them and rep stated that they have not received it. I have tried calling them back chatting with and still luck.

I have also had a hard time with this. I have done all what I was told to do. I even contacted the number given when I was told my stuff was invalid and went through many menus before I was told I had reached "a number that was not in service". I finally had to contact customer service multiple times through chat. I have finally gotten somewhere, hopefully, but will see in a few weeks. The last time I submitted all my paperwork online, it ended up being invalid and I was never informed. Even though it says that you will be contacted.

I think I have finally gotten everything sent in correctly. I'm keeping tabs on my status to make sure. The past two times I have looked it still says pending.

No, I haven't submitted anything through email. I was told to go to VZW.com/switch. I went there and entered in all my information. The first time I was given an invalid, although I was a little annoyed by that, just due to the lack of communication because it said it someone would contact me and they didn't.

I did the same thing the second time, this time I kept tabs on it every few days, I ended up receiving another invalid. It told me to send my "corrections" to an email, but I wanted to make sure I had the correct forms and not wait another 45 days only to see another invalid. So I called the number they provided, I was disconnected by it, it told me I had reached a non-working number after going through the menu options. So I decided to chat with a customer service rep. I asked him what I needed and he told me. He said he changed my status to pending again, so I went behind him and double checked it and it did.

Sorry for the life story, wanted to see if I could help out any.
