Why can't I make calls?

I have a phone that can receive calls fine, but EVERY call placed (whether dialed with 7 digits or 10 digits) is answered:

"Welcome to Verizon Wireless.  Your call cannot be completed as dialed.

Please check the number and dial again.

Announcement 4 switch 94-1"

This phone number was ported a week ago.  Should have been completed by now.

Before anyone on this forum suggests the obvious solutions, they have all been tried:

Good signal strength. 4 of 5 bars.

Have turned phone off, removed SIM, waited, installed SIM, powered on.

PRL was checked and is correct.

No phone SW upgrades are available.

This affects every number dialed.  Even voicemail can't be accessed because it is accessed as a call, and gets the same message.

Verizon had a support ticket [removal required by the Verizon Wireless Terms of Service] for this issue, the technician called Monday but I was unable to answer at the time.

A voice mail (that I can see, but not access!) was left.  I believe Verizon thought the issue was closed, but it is not.

I still cannot place calls.  Is there any way to directly contact the customer support and re-open this ticket.

John, having this experience is not the way we want you to start out with us. I see that you have done many steps already to get the device to do what it needs. To access your voicemail you can call your cell number from any other phone and when your message picks up, press the * key. When prompted dial your password and then you can hear your messages.  This will only work if you have already set up your mailbox. In order to better serve you we would like to get this conversation over to facebook or twitter where we can dig in deeper for you and get this resolved. call from another phone 800 922 0204 to reach tec to make sure your device is registered on network....sounds like device not talking to network and it may be easy fix   let us know if it get fixed soon by calling tec support team

Thanks.  I did get through to a helpful support person.  After working on it a while, and escalating, and bringing on others to look at the problem, it just appears to be a very weird device issue.  Everything was set up correctly on the phone and on the network.

I'm having the same problem exactly.  Every call I make, 7 digits or 10,  gives me the same message:  Your call cannot be completed as dialed.  

Verizon customer service is closed until tomorrow, so my phone is useless for making calls.
