If you plan to port your number to a different carrier, you'd initiate the process with that carrier. Otherwise, you'd call Customer Service or visit your local Corporate Verizon store to cancel service.
Cancellations are effective on the last day of that month's billing cycle. If you bought your phone from somewhere other than VzW, you also may incur an ETF from that entity.
The ETF should be in your final bill, which is mailed to the mailing address on file.
